Multi Choice Questions Incorrectly Marked

Recently my practice exams are having issues with multi choice questions, when marking them it is saying I got them wrong and every answer is selected. The screenshot is an example of this where I chose both of the correct answers but at the review its showing as an incorrect question like this

Could you please tell me if this issue happens with all multiple-choice questions or just some of them?
