Multi Choice Questions Incorrectly Marked

Recently my practice exams are having issues with multi choice questions, when marking them it is saying I got them wrong and every answer is selected. The screenshot is an example of this where I chose both of the correct answers but at the review its showing as an incorrect question like this

I have the same issue doing one of the review mode exams. Questions that were correctly answered at the time seemed to be marked wrong at the review stage. I am answering some questions from a laptop and some from a mobile phone. It is quite frustrating as my result is wrong by quite a bit.
